Hospital and Health Service Integrated Care Programme
Cogility was engaged by a large public hospital to rapidly design the technical components of an Integrated Care Programme being collaboratively developed by the hospital and the region’s Primary Health Network. A full concept of operations along with implementation planning artefacts for the programme was developed in a short period of time.
The Integrated Care Model was designed to allow technical interactions between the new acute care facility at the hospital and the general practices and aged care facilities working in the Integrated Care Programme. In developing a viable environment, a series of technical challenges were addressed, enabling information sharing to occur in line with legislative and policy constraints.
Government is now formalising its support for Integrated Care across the country, with significant investment of funds to harness the success of the trial programmes and to propagate the model across other health organisations.
